Financial Assistance Guidelines 2013

The Brentwood Bay Shore Breast Cancer Coalition has established a new set of guidelines to help create equality and fairness for those we assist financially. Because of the wide variety of situations that women find themselves in, it has been determined that at this time certain regulations are required for funding in order to guarantee that we can help everyone who needs our assistance in some manner.

Women should be in active treatment when receiving assistance. The support group has many members who are in remission and no longer are receiving active treatment therefore those women are no longer eligible for financial assistance except as stated below. Exceptions can be made for individuals without insurance of any kind to assist with payments for chronic medication.  We will continue to recommend families in need for assistance at the holidays from various organizations that reach out looking for qualified “Adopt-a-Family” candidates.

Due to the possibility that if four women utilized the maximum of each category, that it would prevent anyone else from receiving assistance, no one individual can utilize more than $3000.00 per year in assistance excluding transportation during active treatment. Every effort will be made to assist each woman in need to find sources of assistance through the use of appropriate social services. Rent will be paid for the maximum of a 3 (three) month period not to exceed $2500.00 per year.

Utilities (water, gas, oil, or electric) will be paid for a three month period not to exceed $1000 per year.
Medications will not exceed a maximum of $1200.00 per year.

Co-pays and payments toward medical testing inclusive of all radiological and hematological testing will not exceed $1000.00 per year.

Co pays for doctors, hospital admissions, and surgical procedures will not exceed $1,500.00 per year.

Food vouchers will be supplied when needed for the duration of chemotherapy with a maximum of $1,200 per year.

Board members will arrange transportation for chemotherapy and radiation treatments through Choice Ambulette Transport. Use of transportation for doctor visits outside of active treatment will be restricted to $400.00 or 4 trips per year. Active treatment will not be restricted. Women are encouraged to obtain transportation through other means if at all possible.

Durable medical equipment will be paid in full when necessary to include one prosthetic and 2 brasper year and a lymphedema sleeve when ordered per number of mastectomy procedures. (A woman with bilateral mastectomies would receive double the amount). These items must be procured through the contracted SALUMED Pharmacy of Personal Touch Boutique with which the organization has contracts. The Long Island 2 Day Walk is organized to raise awareness and funding for the fight against breast cancer on Long Island. 100% of the donations raised by the participants are distributed to grassroots service organizations so the funds reach the members of the Long Island community affected by breast cancer.

LI2DAY has raised over $4 MILLION from the annual 35-mile, two day walk. Local grassroots breast cancer organizations from Manhasset to Montauk receive funding from LI2DAY for programs that assist with medical expenses, transportation, prosthetics and wigs, physical therapy, child care, research, education and more.
